Mission:

Provide personalized diabetes education to prevent diabetes onset, progression and complications.

About the educator:

Dr. Sarah Kwon is a clinical pharmacy specialist who has established chronic disease management clinics such as multidisciplinary Diabetes Management Clinic, Asthma Management Clinic, and Cholesterol Management Clinic, specializing in drug therapy management.

She became a Board Certified Diabetes Educator (CDE) in 1999 and has given personalized education and counseling to over 1000 individuals with diabetes.  She is also a Board Certified Asthma Educator (AE-C).  Previously at a large managed care health organization (HMO), she established and piloted the first Pharmaceutical Care Service in 1997, a medication management and counseling service, incorporating direct patient care, working with primary care providers and endocrinologists.

She has extensive teaching experience as a clinical assistant professor and preceptor at multiple universities, and has been involved in many oversight committees for chronic disease management. 

She is passionate about opportunities that maximize her love of teaching/education. She is married with two sons and has a golden retriever.  Her other passions are tree house building, photography and water-color.
